PRE‑QUALIFICATION REQUEST TERMS, CONDITIONS & E‑SIGN CONSENT

(Please review carefully and retain a copy for your records.)

  1. Overview

Submitting this Pre‑Qualification Request requires only soft credit inquiries and has no impact on your credit score.
A hard inquiry will only occur after a United Spas representative contacts you and you provide explicit verbal authorization.

This request does not determine loan terms, does not provide any estimated approval amount, and does not constitute a credit application.

  1. Purpose of This Request

You are providing written instruction under the Fair Credit Reporting Act (“FCRA”) for United Spas MFG Co. and its designated finance source, Aqua Finance, Inc., to obtain and use information from your personal credit profile or other information from credit reporting agencies solely to evaluate your eligibility to pre‑qualify for financing for a United Spas hot tub.

A United Spas representative will contact you after submission to review models, pricing, and next steps.

  1. No Estimated Approval Amount Provided

United Spas MFG Co. does not provide:

  • Estimated approval amounts
  • Estimated loan terms
  • Estimated interest rates
  • Any guarantee of financing

Your information is collected only so a representative can review your request and discuss options with you directly.

Final terms are determined solely by Aqua Finance after you authorize a formal credit application.

Submitting this request:

  • Does not run your credit
  • Does not create a hard inquiry
  • Does not affect your credit score
  • Does not obligate you to purchase or finance anything

A hard inquiry will only occur after:

  1. A United Spas representative contacts you,
  2. You review pricing and model options, and
  3. You verbally authorize us to submit a formal credit application to Aqua Finance.
  4. Information You Provide

You certify that all information you provide—including your name, address, contact information, date of birth, Social Security Number, employment details, and income—is true, complete, and accurate.

Your information is subject to verification by United Spas and Aqua Finance.

Any false, incomplete, or unverifiable information may result in denial or changes to potential eligibility.

  1. Use of Your Information

Your information may be used for:

  • Identity verification
  • Pre‑qualification review
  • Fraud prevention
  • Contacting you regarding financing options
  • Preparing a formal credit application (only after verbal consent)

Your information will not be sold or shared with any unrelated third parties.

  1. Finance Source

United Spas MFG Co. uses one finance source for consumer hot tub financing:

Your information will only be shared with Aqua Finance after you verbally authorize us to proceed with a formal credit application.

Pre‑qualification is invalid for:

  • Applicants under age 18
  • Applicants under age 19 in Alabama
  • Purchases intended for commercial use, rental, or export

You expressly consent that United Spas MFG Co. and Aqua Finance may contact you for any purpose related to your request or application using:

  • Phone calls
  • Text messages
  • Emails
  • Electronic messages
  • Live or prerecorded/artificial voice messages
  • Automated dialing systems

Your consent applies to any phone number or email address you provide, including mobile numbers that may incur charges.

You agree that calls may be monitored or recorded.

  1. Data Security & Limitation of Liability

United Spas MFG Co. uses commercially reasonable safeguards to protect your information. However:

  • United Spas is not liable for unauthorized access, cyberattacks, data breaches, or interception of information by third parties

Married Wisconsin Residents: No marital property agreement or court decree affects the creditor unless provided before credit is granted.

Ohio Residents: Ohio laws require equal credit opportunity. The Ohio Civil Rights Commission enforces compliance.

New York Residents: You may request disclosure of whether a consumer credit report was obtained and the name of the reporting agency.

Rhode Island & Vermont Residents: A consumer credit report may be obtained for review, renewal, or collection purposes.

UNITED SPAS MFG CO. — E‑SIGN CONSENT AND AGREEMENT

This E‑Sign Consent applies to any pre‑qualification request or credit application you submit to United Spas MFG Co.

Certain laws require us to provide information in writing or obtain your signed authorization. By agreeing to this Consent, you allow us to:

Electronic agreements carry the same legal effect as signed paper documents.

You may request paper copies at no charge. Paper copies will be mailed to the address we have on file.

You agree to provide a valid email address and notify us promptly of any changes. Electronic communications will be considered delivered if sent to the email address on file.

  • You have read and understand these Terms, Conditions, and E‑Sign Consent
  • You agree to receive communications electronically
  • You authorize United Spas MFG Co. to share your information with Aqua Finance only after verbal consent
  • You understand that no credit check occurs until you verbally authorize it
  • You understand that no estimated approval amount will be provided through this online request
